Need your Out-of-State Immunization Records transferred for school?
The following immunization services are offered:
Childhood Immunizations needed for School Entry (680 Form):
Out-of-State Records transfers MUST be dropped of in person by a parent, Monday through Friday during normal clinic hours. The same parent who drops off the record request MUST be the same parent who picks up the completed 680 Form.
Please call DOH – St. Johns if you wish to schedule an appointment.
If you need record transfers sooner, check with your local pediatrician's office. Many local urgent care centers, minute clinics, and pharmacies also provide this service.
