There are three ways to request records transfer. No appointment is required for this service.
In Person
Record transfers can be dropped off in person by a parent/guardian Monday through Friday from 8 a.m. to 5 p.m.
Fax
Transfers can be faxed to us at 904-825-6875. Please be sure to include the immunization history, DH680 request form, and a copy of your government-issued ID.
