Out-Of-State Immunizations Records Transfers
Immunizations for School Entry (DH680 Form)
There are three ways to request records transfer. No appointment is required for this service.
- In-Person
Record transfers can be dropped off in-person by a parent/guardian, Monday through Friday, 8 a.m. to 5 p.m. - Fax
Transfers can be faxed to us at 904-825-6875. Please be sure to include the immunization history, DH680 request form and a copy of your government-issued ID. - Email
Record transfers can be emailed to StJohnsImmu@FLHealth.gov. Please be sure to include the immunization history, DH680 request form, and a copy of your government issued ID. All forms should be sent in PDF format.
Religious Exemptions
