The Florida Department of Health’s Bureau of Family Health Services needs your help to gather information about the health care needs of women, children, and their families, including those with special health care needs.
The purpose of the surveys is to identify available services in your area and address
solutions to community health care challenges.

Caregiver Survey
For parents and caregivers of
a child, youth, or young adult
with special health care needs.
Youth Survey
For youth and young adults
(if under 18 years of age, you
will need to provide parental
consent within the survey).
Deadline to complete survey is
February 12, 2025.
The data being collected from the survey will provide the Bureau of Family Health Services with valuable
feedback for long-term program planning that supports the health needs of Florida’s women and children.
