Appointments are Required for Immunization Services

Immunizations services are scheduled by appointments only. If you would like to make an appointment, please call 904-506-6081 option 2.

Bring the following with you to your scheduled appointment:

  • Valid photo I.D. (such as a driver’s license)
  • Your child’s insurance card

Children from birth through 18 years of age are eligible for immunizations. VFC immunizations are provided to children at no cost to the parent/guardian. There is an administration fee of $5.50 per immunization that will be billed to your insurance/Medicaid plan. No child will be prevented from receiving immunization services due to inability to pay.

Immunizations Available

DOH-St. Johns offers the following immunizations for children:

  • Diphtheria, tetanus, and acellular pertussis (DTaP)
  • Haemophilus influenzae type b (Hib)
  • Hepatitis A (HepA)
  • Hepatitis B (HepB)
  • Human papillomavirus (HPV)
  • Influenza vaccine (IIV4)
  • Measles, mumps, and rubella (MMR)
  • Meningococcal ACWY (Menveo)
  • Meningococcal B (Trumenba)
  • Pneumococcal conjugate (PCV15)
  • Poliovirus (inactivated) (IPV)
  • Rotavirus (RotaTeq)
  • Tetanus, diphtheria, and acellular pertussis (Tdap)
  • Varicella vaccine (Var)

Combination Immunizations

  • DTaP, hepatitis B, and inactivated poliovirus vaccine (Pediarix)
  • DTaP, inactivated poliovirus, and Haemophilus influenzae type b (Pentacel)
  • DTaP, and inactivated poliovirus (Kinrix)
  • DTaP, inactivated poliovirus, Haemophilus influenzae type b, and hepatitis B (Vaxelis)
  • Measles, mumps, rubella, and varicella vaccine (MMRV)

Out-Of-State Immunizations Records Transfers

Immunizations for School Entry (DH680 Form)

There are three ways to request records transfer. No appointment is required for this service.

In-Person
Record transfers can be dropped off in-person by a parent/guardian, Monday through Friday, 8 a.m. to 5 p.m. 
 
Fax
Transfers can be faxed to us at 904-825-6875. Please be sure to include the immunization history, DH680 request form and a copy of your government-issued ID.
 
Email
Record transfers can be emailed to us at StJohnsImmu@FLhealth.gov. Please be sure to include the immunization history, the DH680 Request form and a copy of your government issued ID. All forms should be sent in PDF format.

Religious Exemptions

In-Person
