Protocols for Controlling COVID-19 for the 2021-2022 School Year

  • The CHD screens incoming COVID-19 Cases by age.
  • The CHD prioritizes cases (known or suspected) for infants to children up to 18 years of age.
  • The CHD has established a direct phone line for Schools to leave messages for CHD Case Investigators with an expectation of a call-back within 24 hours.
  • The CHD has established a second direct phone line for Parents to report that their Student has a positive COVID-19 test result which can be reached at 904-506-6217 with the expectation of a call-back within one (1) business day .
  • A Joint Information COVID-19 Quarantine Call Center is available to provide answers to Parents of Direct Contacts - 904-819-1101.
  • The CHD will report lab-confirmed and/or probable COVID-19 cases (students) to the School District’s Point-of-Contact at a predetermined frequency daily.
  • The School District will notify the CHD of any COVID-19 Positive Staff at a pre-determined frequency daily via the School District’s Point-of-Contact.
  • The School District will identify and notify the School-associated Direct Contacts of COVID-19 positive Students and Staff.
  • The CHD provides the School District with guidance for use with Direct Contacts of COVID-19 Positive Students and Staff so the School District can complete the initial notification to those individuals.
  • The School District will provide the CHD with a List of newly identified Direct Contacts at a pre-determined frequency. The List will include the following: (1) name of the case to whom the student or staff member is a direct contact; (2) the direct contact’s full name, DOB, phone number, home address; (3) the school and classroom(s) of the direct contact; and if applicable, (4) the last day of attendance.
  • The CHD will make every attempt, given staffing limitations, to communicate with positive cases to conduct and complete the case investigation process.
  • The CHD offers COVID-19 diagnostic testing resources to symptomatic students and staff as well as identified direct contacts (students and staff) of lab-confirmed COVID-19 cases, Monday through Friday, at the CHD.
  • The CHD provides linkages to testing resources throughout the community.
  • The CHD offers COVID-19 Vaccinations to school students and staff, Monday through Friday, at the CHD.
  • The CHD provides information and linkages to vaccinations sites throughout the community.
  • The School District will manage the return to school of asymptomatic direct contacts.
  • The CHD will make a determination of eligibility for clearance/return-to-work for COVID-19 positive staff assuming such staff could be reached by the CHD and the COVID-19 positive staff member participated in the investigation process. Once cleared, the CHD will notify the School.
