ST. JOHNS OFFERING BACK-TO-SCHOOL IMMUNIZATION SERVICES
St. Augustine - School starts on August 10th this year, and the Florida Department of Health in St. Johns County (DOH-St. Johns) is encouraging parents to have their children immunized now to avoid the back-to-school immunization rush.
If your child is due for any immunizations, please visit their health care provider to make sure they are ready for the upcoming school year. DOH-St. Johns also provides no cost immunizations for eligible children and is offering walk-in immunization services (no appointment required):
WHAT: Back-to-School Immunization Services at the Health Department
WHERE: 200 San Sebastian View, St. Augustine, FL 32084
WHEN: Monday – Friday (from July 13, 2015 thru August 12, 2015)
TIME: 8:00 AM – 12:00 noon and 1:00 – 5:00 PM
Each child must be accompanied by a parent or legal guardian, and please remember to bring your child’s current immunization history, a valid state-issued photo ID, and any applicable insurance cards.
If you are new to the state of Florida and need to have your child’s shot records transferred onto a DH-680 form for admittance to school, starting Monday, July 13th, DOH-St. Johns will be offering a free drop-off service with no waiting. It’s quick and easy! Simply:
- Bring your child’s current immunization history and a valid state-issued photo ID to the Health Department, Monday – Friday between 8:00 AM to noon, and 1:00 – 5:00 PM.
- A nurse will import your child’s immunization history.
- We will call you within 48 hours to pick up your completed DH-680 form.
- If your child needs immunizations to meet school entrance requirements, we will notify you so you can return with your child to get the required shots.
