St. Johns County Health Leadership Council Launches Fifth Community Health Assessment Survey

—Requesting Feedback from St. Johns County Residents—

ST. AUGUSTINE – Led by the Florida Department of Health in St. Johns County since 2004, the St. Johns County Health Leadership Council is working to complete its fifth comprehensive Community Health Assessment and Community Health Improvement Plan which will be published later this year.  Your feedback is important, please complete our survey: How Healthy is St. Johns County?

An important component of the Community Health Assessment process, a community-wide survey will be conducted throughout May and June.  This is an opportunity for St. Johns residents to share their opinions about health and the quality of life in the county. 

Your input will continue to help make St. Johns County a healthier place to live, learn, work, and play.  Paper surveys will be offered at numerous locations throughout the county.  A web-based survey is also available online:

St. Johns County has remained the healthiest county in Florida for the last six years, according to the Robert Wood Johnson Foundation and the University of Wisconsin’s Population Health Institute’s national 2017 County Health Rankings Report – and we want to keep it that way!  The information collected in this community-wide survey will be used to develop the 2017-2020 Community Health Improvement Plan for St. Johns County which will address opportunities for improvement identified during this community health assessment process.

St. Johns County Health Leadership Council, chaired by Florida Department of Health in St. Johns County and Flagler Hospital, is a collaborative of community partners dedicated to county-wide community health assessment and health improvement planning. The Council’s mission is to promote, protect and improve the health of all people in St. Johns County, Florida.
