The Florida Department of Health in St. Johns County (DOH-St. Johns) Epidemiology Service Center is responsible for the investigation, monitoring, and control of reportable diseases and conditions in St. Johns County. The Department serves as a public resource for information and education about specific diseases, the epidemiology of those diseases, and their prevention. 

Diseases or conditions listed on the  Reportable Diseases/Conditions in Florida (PDF, 366 kB)  form, dated October 20, 2016 are of public health significance and are required to be reported to the Florida Department of Health.

If any of these diseases or conditions are identified in a St. Johns County resident, the practitioner, hospital, laboratory, or other individuals identifying or diagnosing the condition should notify DOH-St. Johns Epidemiology via telephone at 904-506-6081, by fax at 904-823-2380, or other confidential means of communication, dependent on the disease.

Please refer to the Helpful Links for timeline reporting guidelines.  Disease Reporters may report clusters or individual cases of diseases and/or conditions not listed by rule.  The DOH-St. Johns Epidemiology Service Center investigates clusters of illness commonly known as outbreak investigations, in conjunction with our Environmental Public Health Service Center.
